Instructions for RegistrationAll users of SR3 must register before reagents can be requested or deposited, and this registration is effective for five (5) years from the date of approval. You will be issued an account number which must appear on each Reagent Request Form and Reagent Deposit Form . SR3s staff will be available to answer questions and assist with the registration process. Before you mail your Registration Form to the SR3, make sure of the following conditions: 1. Obtain the appropriate, original institutional signatures. Where the application forms specify "Duly Authorized for Depositor's Institution", signatures must be obtained from an official capable of legally binding your Institution. 2. Check that your registration documents contain ORIGINAL signatures. The Schistosome Related Research Repository cannot accept registration forms by FAX. You can fill out your registration form on-line, but you will need to print it out and sign where indicated. You must MAIL your Registration Form with the original signatures to the address provided below. 3. Determine which of the three Indemnification agreements is most appropriate for your institution and submit only ONE of them. The three choices are: • Standard Indemnification Agreement – to be used by most registrants • State Institution Compliance Agreement – if you work for a U.S. State Institution that will not sign the Standard Indemnification Agreement • Waiver of Indemnification Agreement - if your institution cannot sign the Standard or State agreements • U. S. Government employees are not required to submit an Indemnification Agreement.
